We are accepting grant applications during the months of May through August 2021
and issuing a grant the end of September 2021. For grant applications to be
considered, they must be postmarked by August 31, 2021.
To apply for a grant to assist with medical equipment expenses, please print off the
application below and follow the instructions on the form.
